Using the M4S MicroChip Registration form in DVMAX

The following article will guide you through loading the M4S Microchip Form into DVMAX and it can be used as your Registration Form for M4S Microchip Solutions.   Once the Document is loaded in to your DVMAX, you can simply print this document from within the patient record.

Step 1: Download the M4S Chip Registration Form

  1. Click the following link to download the document:

  2. Save the file to your Desktop.

 

Step 2: Load and Save the Document Template into DVMAX

  1. Open any patient record in DVMAX.
  2. Click on the Word Processing tab.
  3. Go to the File menu, choose Open.
  4. Click on the Disk.. button.
  5. Navigate to the Desktop and find the file you previously downloaded.  Choose to open the document. This brings the document onto your WP screen.
  6. Go to the File menu, choose Save As.
  7. Type at least one asterisk, then name your document.  Ensure that the Document Class is Protected Document.
    For Example: **M4S Microchip Registration Form
  8. Click the Save button.
